Oakland County issued the following announcement on Feb. 11.
A news conference to announce a historic and comprehensive transformation of health care services with a focus on women, children and families in Oakland County which has nearly a quarter million residents who are either uninsured, receive Medicaid benefits, or are on a marketplace exchange health plan.
Oakland County Executive David Coulter will announce a partnership with Honor Community Health and Oakland Livingston Human Service Agency to offer a comprehensive, integrated health care model in the county’s Pontiac and Southfield clinics. Phase one services will include general primary care, family planning services, and dental care. When complete, this partnership will include mental health services and important supportive services, including access to insurance coverage, Head Start early childhood education, and emergency services such as energy, food, and housing assistance. This patient-centered, data-driven, continuum of care is needed to address barriers that impact chronic disease, infant mortality, and life expectancy rates.
